Physical Assessment
[I]n nursing terms assessment is seen as the first stage of the nursing process in which data is obtained and collected and from which a nursing care plan can be devised. Assessment is a multi-stage cyclic process viewed as the essence of nursing and involves initial ideas which are formed about clients’ existing health needs followed by confirmation of the data obtained, clarification and understanding, reassessment and evaluation (Aggleton and Chalmers, 2000). Assessment and the use of the nursing process are embedded within nursing and together they constitute a standard proficiency for pre-registration nursing programmes at the point of registration.
